Assumption of Risk
I, the undersigned, acknowledge that participating in archery involves certain risks, including but not limited to physical injury, accidents, and damage. I understand that archery requires focus, coordination, and the use of equipment that can be dangerous if misused. I understand that my child will be supervised at all times during the activity, but there are inherent risks involved.
Release of Liability
In consideration of the opportunity for my child to participate in archery, I hereby release, discharge, and hold harmless [Dee’s Archery Events], its employees, agents, contractors, and volunteers, from any and all claims, liabilities, damages, and expenses arising out of or in any way connected with my child's participation in the archery activity, including any injuries sustained during the activity. This release includes, but is not limited to, claims arising from negligence.
Medical Authorization
In the event of an emergency, I hereby authorize the event organizers to seek medical attention for my child if needed. I understand that every effort will be made to contact me or my emergency contact in the event of a medical emergency.
Acknowledgment of Safety Procedures
I affirm that I have reviewed all safety rules and guidelines provided by the event organizers, and I understand that my child is expected to follow all rules for safe participation in the archery activity.
Consent
By signing below, I acknowledge that I have read and fully understand this waiver and release of liability. I understand that my child's participation in this archery activity is voluntary, and I consent to my child's participation.
